Filling out your medical forms is a matter of sitting down and doing it

What is Patrol Z?Ahh, medical forms. They’re always a pain to finish filling out.

And that due date always seems to sneak up on us.

Speaking of which, the deadline for turning in medical forms for the 2013 National Scout Jamboree is April 15, so it’s time to get rolling.

Don’t worry.

If you’re freaking out about how to get it done, I have the solution.

Medical forms for the jamboree are not as complicated as they might seem. In fact, they’re quite easy to figure out.

To get started, log in to your account  to see your jamboree registration dashboard. There, you should see a button at the top that says “Medical Form Entry.”

Then, follow these three steps and you’ll be golden.

Step 1.

Have your parent or guardian fill out parts A and B of the form. Once complete, print out the form.

Step 2.

Take your printed form to your physician to fill out and sign off on part C.

Step 3.

Once your form is complete, mail it to:
